Program Description:
This form is for new or transfer students who have an I-20 or DS-2019 with a Fall 2020 program start date, but wish to start their academic program outside the U.S. for the Fall 2020 semester. If you want an adjusted I-20 or DS-2019 with a Spring 2021 start date, please complete this form.

If you are a continuing student, DO NOT complete this form.
